PRIORITY UNWANTED EVENTS (PUEs)

CRITICAL CONTROLS

Contact with moving or rotational parts of machinery

  • Isolate and lockout and test for dead.
  • Machine guarding on all rotational and/or moving parts.
  • Barricading of areas to prevent unauthorised access where guards are removed.
  • Trip wires, emergency stops and start-up alarms in working order.
  • Trained, competent and appointed operators.

Persons falling from heights

  • Certified Fall Protection Equipment (Safety Harness).
  • Accredited working at heights training.
  • Structures (Silo, Drum, bins etc.) and tanks are fitted with handrails, safe walkways, access and ladders.

Unsafe movement of plant and equipment

  • Operators to ensure hand brakes are engaged, stop blocks placed and keys removed. (Isolation, lockout and key control.)
  • Approved Traffic Management Plan.
  • All operators/drivers must be licensed, trained and authorised.
  • No long-distance driving between 22:00 and 04:00 without the required permission. (LDV and Trucks.)
  • Reverse hooters in working condition.

Contact with hot bitumen

  • Wear correct PPE – fireproof balaklava, heat resistant overalls, boots and gloves, face shield/goggles leather apron and spats.
  • Use of splash guards at all bitumen offloading points.
  • Trained and authorised bitumen attendants.
  • Emergency shower must be functional.
  • Bitumen must be sampled at the sampling point.

Loss of load during lifting operations

  • Competent and approved subcontractor and/or operators appointed for lifting operations before work commence.
  • Lifting equipment inspection, certification and tagged – Quarterly-(LMI).
  • Trained, competent and appointed operators.
  • Appropriate lifting equipment to be used for the task at hand. (Jacks, Trestle, etc.)
  • Physical barricading of drop zone includes the use of guide ropes during lifting operations.
  • Load test certificate valid and maintained (forklift, skip, etc.). Skips to be used in accordance with relevant specifications.

Fire and explosions

  • Ensure proper earthing of structures.
  • Fire fighting equipment.
  • Acetylene/gas welding-flashback arresters to be fitted onto the equipment.
  • Emergency preparedness and response plan in place.
  • Hazardous chemical substances not to be stored together with combustible materials.
  • Toluene extracting rooms-ensure room is well ventilated with a fan placed at floor level to ensure there is no build-up of toluene vapors. Doors are sealed properly when closed. Use of flame proof plugs/move plugs outside.
  • Keep any source of ignition away from vents and fuel pipes.

TASK AND ACTIVITIES

I will ONLY operate or carry out activities for which I am trained, competent and authorised to do.

OPERATING MACHINERY

I will NEVER drive or operate mobile machinery while using my cellphone and other electronic devices.

ISOLATE, LOCKOUT AND TAG

I will ALWAYS isolate, lockout and test for zero energy before any work is carried out on electricity or moving machinery such as conveyor belts.

CRITICAL CONTROLS

I will ALWAYS stop work if critical controls are not in place.

MACHINE GUARDS

I will NEVER use equipment without the necessary machine and equipment guards.

LIFTING OPERATIONS

I will ALWAYS plan lifting operations and control the area.

LIFESAVING RULE – SITUATIONAL AWARENESS

I will ALWAYS keep myself and others out of the line of fire; uncontrolled release of hot bitumen and uncontrolled release of fumes.
